Please remember to bring your completed forms with you for your first appointment. If you have any questions or problems getting these files to print properly, please call our office and we will gladly fax or mail them to you.
For patients less than 18 years of age, a parent or guardian must accompany them to assist in providing an accurate medical history and giving consent for treatment.

Payment is due on the day professional services are rendered. At the end of each appointment, we will submit your dental claim to your insurance company. Most insurance companies will respond to a dental claim within 2-7 business days. A response to an estimate or predetermination from an insurance company may take up to six weeks.
For your convenience, we accept Visa, MasterCard, Interac, cash, and cheques. Please remember you are fully responsible for all fees charged by this office, regardless of your insurance coverage.
Jeff Weaver Dental makes every effort to provide you with the best care. Our team will work closely with you to maximize your insurance benefits/reimbursement. As there are many different dental insurance policies, we do our best to provide you with the most accurate estimate based on the insurance information you provide us. As a courtesy to our patients with dental benefits, we are happy to create and electronically submit dental claim forms and predeterminations to your insurance company. Some forms may have to be mailed to your insurance company. Please bring your insurance information to your first visit so we can help process your claims as quickly as possible. Reimbursements are sent directly to you from your insurance company.
The safety of our patients and staff is our top priority. To protect our patients and ourselves, we strictly maintain sterilization and cross-contamination processes using the standards recommended by the Royal College of Dental Surgeons of Ontario.
We use ultrasonic cleaners and the Miele Dental Washer Disinfector to clean our instruments. Following disinfection, our instruments are wrapped and sterilized in an autoclave. Daily spore tests of our sterilizers ensure that they are functioning properly.
